Examination reveals no physical abnormality of the eyes and in particular the pupil reactions are normal. If total blindness is claimed a difference may be noted in the patient's behaviour, for example in navigating a room full of furniture, depending on whether the patient is aware of the presence of an observer. Decreased visual acuity in one or both eyes tends to be inconsistent and only the top letter on the Snellen chart may be read, irrespective of the distance. Visual field loss is similarly inconsistent even during the course of a single examination.
Cicatricial This may be caused by trauma, especially burns, or tumours involving the skin of the eyelids. Treatment is surgical but temporary measures to protect the cornea may be required. Trichiasis Misalignment of the eyelashes is a consequence of scarring of the eyelids margin from infection or trauma. Treatment is required if the lashes abrade the cornea. Simple epilation gives temporary relief but permanent removal by electrolysis, cryosurgery or surgical transplantation of the lashes is necessary.
Chronic conjunctivitis Chronic allergic conjunctivitis can cause symptoms ranging from slight pricking discomfort to intense irritation. A less common cause of persistent conjunctivitis is viral infection of the lid margin, for example by the molluscum and papovaviruses. Keratoconjunctivitis sicca (see Chapter 9) causes chronic ocular discomfort. In the milder forms the eyes may appear normal but special investigations reveal tear deficiency. Poor lid closure This is usually the result of facial palsy or severe proptosis.
